I would expand some of these:

Title should match h1 tag exactly. Description needs to be readable to humans. Meta Keywords aren't used by google.

It's quite difficult to sum up what search engines look for in a neat list. There is other criteria, such as page layout, that affects SERPs. Knowing about keyword phrases or keyword density isn't enough, you need to know how and when to use keywords.

If I had to sum up SEO it would be: relevance. Everything on the page and links coming into the page and the pages they come from need to be relevant to the topic.
